Are there any sensible dietetic measures which can be taken in gastroenterology?

U. Rabast, Hattingen

For more than a century, dietary therapy in normal clinical treatment has been influenced by the concept of sparing specific organs. Stomach, intestinal, liver, bile and pancreas diets are intended to spare the secretion and/or motoric activity of the corresponding organ, thus favouring the process of cure or improvement.

There is no scientific basis for these measures, which are purely empirical. The German Working Party for Clinical Nutrition and Dietetics carried out a questionnaire in 1978 on almost 2000 hospital patients. This gave the result that a relatively high proportion of patients suffered symptoms of consuming specific foods – independently of any disease. Taken together with earlier scientific articles, the results of this questionnaire raised doubts about the practice of dietary therapy.

The findings have led to the elimination from normal clinical treatment of organ-specific forms of bland diets with no scientific basis and these have now been replaced by the “light balanced diet” or the “basic gastroenterological diet”. The diets that are now used are based on scientific evidence and are an essential component of the treatment of several clinical presentations, to which there is often no alternative .
